Starting May 1, 2023, Hub Transit will have two fare tiers for its On-Call bus service. (Formerly the On-Demand bus.) These are a full rate for riders without disabilities and a reduced rate for disabled riders.

To qualify for the reduced rate, riders must present a valid Disability Card to the Hub Transit driver each time they board the bus. It will look similar to this:

To apply for a Disability Card, please complete the form below and bring it to the Municipality of Sioux Lookout office at 25 Fifth Avenue.

Appointments are required, and you should expect processing to take about 20-30 minutes.

To schedule a time to get your Disability Card, please call the HR Administrator at 807-737-2700 extension 2225. Office hours are Monday to Friday from 9:00 a.m. to 4:00 p.m. A photo will be taken and you will leave with the card.*

Printed copies will also be available at the Customer Service desk at the Municipality of Sioux Lookout Office, 25 Fifth Avenue, during regular office hours. Appointments are still required to complete the application process to ensure staff availability.

* If a referral from a medical professional is requested, processing will not be completed until a signed form is returned to staff.
